OC Transpo OC Transpo provides transit services across Ottawa and into Gatineau. Serving nearly 340,000 riders per day, the operate a fleet of over 700 vehicles. Phone: 613-560-5000
Para Transpo Para Transpo is a shared-ride, book-in-advance, public transit service. The service is open to eligible customers who are unable to take conventional transit (buses and O-Train) some or all of the time due to a disability. It is funded by the City of Ottawa and administered by OC Transpo. Para Transpo provides minibus and contracted taxi services in Ottawa’s urban communities. - The Snow Go program provides a matching service for older adults and people with disabilities to hire an individual or contractor to clear snow from private driveways and walkways at an affordable rate.
- The Snow Go Assist program provides financial assistance to eligible low-income older adults or persons with disabilities to help with their snow removal costs.

Home and Community Care Support Services Champlain | A wide range of health care services and resources covered through the Ontario Health Insurance Plan (OHIP) can be provided in a patient’s home or at a nursing clinic. Based on assessment, the following services may be available: nursing personal support, physio, OT, long-term care placement, etc. To contact Ontario Health atHome, seven (7) days a week, 365 days a year call 310-2222 (no area code required). This service will automatically direct your call to the nearest office from your calling location. |
Regional Geriatric Program of Eastern Ontario | Regional Geriatric Program of Eastern Ontario provides clinical services for older adults over 65 years of age. Programs include community and in-hospital services. Older adults are referred to Geriatrics for a variety of concerns such as: Recent changes in physical, mental or functional abilities, changes in memory and/or mood. Major changes in support needs, caregiver stress and future planning. Safety concerns – physical, psychological, social, environmental. Ontario Caregiver Organization | The Ontario Caregiver Organization (OCO) exists to support caregivers who provide physical and emotional support to a family member, partner, friend or neighbour. They are one point of access to information, so caregivers have what they need to be successful in their role (24/7 Helpline, peer support, webinars, group and individual counselling, learning library, etc.). West End Integrated Fall Prevention Program | The program provides assessment and intervention for older adults at high-risk for falling. Referrals are accepted from older adults who meet certain criteria. As well, to access the program, older adults must live within the PQCHC area or the Western Ottawa Community Resource Centre catchment area. Champlain Screening Outreach Program (ottawahospital.on.ca) | The Champlain Screening Outreach is a new program enabling those without primary care providers to access cancer screening. The Nurse Practitioner offers cancer screening consultations, assessments, results follow-up, and referral to treatment (if needed) to residents of the Champlain Region (the entire city of Ottawa) who are not currently attached to a primary care provider. Patients can self-refer to the Champlain Screening Outreach by calling 1-833-551-4125 or by booking an appointment online. Your first appointment will be a phone call.
